Order Form

This order form requests all the information I need to create the online memorial for your loved one. Please fill it out as detailed as possible to avoid any unnecessary back and forth during this difficult time arranging everything for their funeral.

You can either upload, email or WhatsApp me all the images and any screenshots you might have of verses etc.
Please write a note in the section about the text for the memorial, if you are sending screenshots and in which order they should appear on the memorial.

You will receive a link of the design draft to approve or request any changes, if needed. Only once you are 100% satisfied with the design I will publish the online memorial and send you the digital memorial card with QR code and link to be shared to your friends and family.
